2023 Interfaith Health & Wellness Summit Grant Application

Purpose of grant:

Anthem recognizes that houses of worship and faith-based organizations are trusted entities, providing spiritual refuge and a place of hope and help during good and challenging times. We appreciate and value your work and are here to support your health and wellness initiatives that are helping improve the communities you serve.

The 2023 Interfaith Health & Wellness Webinar/Summit Grant aims to provide financial assistance to a house of worship or a faith-based organization that implements a health and wellness program for their faith assembly or local community. The program must be implemented between 01/01/24 – 12/01/24.

Grant recipients may become eligible for an It is Well Community Star Award if their program meets one of the following requirements and demonstrates measurable results:

Addresses state health priority:

· Increased childhood immunizations

· Reduced opioid/substance use

· Reduced infant mortality

· Reduced smoking rates

· Reduced obesity

Target populations:

· Rural communities

· Unemployed/under-employed

· Diverse communities

· LGBTQ

· Individuals with disabilities

· Moms, kids, and families in the foster system

· Re-entry of individuals from incarceration

Grant request amount: Up to $1,000
